Key Dates
- Week of April 14: Letter to all PCC foster homes introducing
the census
will go out from Secretary Miller's Office. - April 21: Census list of children in out-of-home care will be set
- April 25: Census list of children in each agency will be sent to PCCs
- April 30: Data entry site will be ready for PCCs to enter data and prepare
and send out census forms - May 7: Several foster homes and a few residential settings need to be ready to go to be assigned at the training and visited starting May 12.
- May 9-10: Training for the census takers.
- May 12: Census visits begin
- May 23: Target date for all PCCs to have all child census forms completed
and mailed to foster homes and residential settings. - August 1-2: Exit conference with preliminary results

If you download the forms listed below to use, you MUST photocopy the second page onto the back of the first page so that it pages one and two are on on the front and back of the same piece of paper. This is necessary for IRB regulations and no payment will be given for visits in which the consent forms are not printed correctly.

- PCC FP Cover Letter - This is the cover letter for the Child Census Form packets. Please print it onto your own letterhead (if you can), personalize it with the name of the foster parent, and sign Ruth's name for her. This has also been approved by the IRB so please do not edit it in any other ways.
